Once you get a feel for the game, I think its a great thing to do. Especially if playing through the game normally gets too easy. Its nice to have a challenge. Trying to play the game with only three hearts also causes you to put more technique into the game, which makes you a better Zelda...

Majora's Mask was the first Zelda game I ever played, and its the game that got me into the series. Its really a toss up between the two for me, but I just enjoy OOT's story more, so I'd say OOT is my favorite.
I'd say Ganondorf from Oot, maybe just because its how I first got to know the character. Hadn't yet played any Zelda games with him in it. His attitude also seems a bit different from himself in any of the other games.

I'm not really sure why people would think Twinrova was difficult. All you had to do was stand there, hold up your shield, absorb and unleash her attacks, then slash. Pretty basic.
Can't remember what my time was for my initial play through. Probably pretty bad as I just messed around/explored and stuff. It took me 31 hours for my three heart challenge.

Maybe this isn't really a plausible idea, but I think that the inside of the clock tower isn't part of Termina. Hence why time moves anywhere else in the land except for when you enter the clock tower.
I'm sure any Zelda fan wouldn't be happy with any of those options. I'm sure they'll always stick something around traditional when making a Zelda game. Or at least, I'd hope they would.
