My worst death, which I have to say is very different from my funniest/dumbest death, would have to be in Ocarina of Time in which I was fighting Dark Link. You know what happened? Well, I was hacking and slashing my way through him (I used to know this technique to defeat him, but in recent years I forgot it). I was getting rather impatient, playing a minimalist-esque run, so I started FLAILING wildly! Well I erroneously stabbed at him and of course he hops on my sword and slashes me.

From there, I was treated with a beautiful GAME OVER. :(

1991. I had valiantly battled from the gates of the Great Palace in AoL though the perilous corridors filled with traps and monsters. For three days I had mapped and planned the proper route. In a brilliant display of skill that I would not truly possess for another decade, I slew the mighty Thunderbird with nothing but my courage and a 12 in sword. Then, I stood before my own Shadow, the last obstacle to the Triforce and bragging rights at lunch for being the first to conquer the game... and had one hit left.

I think you know how this story ends.

Also on Twilight Princess with Epona, but mine was a glitch.

I was doing the escort mission with Ilia and Telma and got to the bridge at the Kakariko Gorge. Here I am, thinking, "hey, I did really good this time, everything's going great!" and then, for some reason, I had to stop and get off Epona to fight. Well, I clicked "A" to dismount, and Link did... but then he jumped WAAAAAAAAAY over Epona and off the bridge, to his doom! I still have NO idea how he did that. Like, I'm not talking a normal Link jump, I'm talking insanely glitchy huge leap unlike anything else!

Jumping across a pitfall in Zelda II only to have something hit me and knock me back, causing me to fall to my death. (Yeah, that happens a lot.)

This. Especially those darn horse/medusa-heads, and the Octoroks, to a lesser extent. For a more recent example. I allowed the rolling stone in the Earth Temple from SS to squash me, and then it happened again the next time I played... :(

This as well, except the objects decided to play ping pong with me. I seriously got hit three or four times before I was sent spiraling to my doom.

Funny: Having the Earth Temple boulder hit me in midair  Not sure how that happened...

Irritating: I was on the last boss in The Gauntlet (OoT 3D) and I had 1 heart, a blue potion, and 1 or 2 hits from winning. The controls happened to lag and... draw your own conclusions. Sooooooo much **************************************.

To this day, I have the hardest time beating BigOcto in OoT. XD He's only a mini-boss.

I don't have too much trouble beating him, but beating him without damage is another story. I do think he's one of the more challenging mini-bosses due to suddenly changing direction.

Bongo Bongo is the one that still eludes me even after several OoT playthroughs. I still find myself taking significant amounts of damage and have to rely on potions to get through the battle. The other bosses don't give me close to the amount of trouble Bongo Bongo gives.
