The captain's hat is an item found exclusively in Majora's Mask. The Mask is a reward for completing the battle with Skull Keeta (AKA "Captain" Keeta) in Ikana graveyard. The mask itself is a replica of Skull Keeta's face & the hat he wore only smaller.

When wearing this mask, Link has the ability to talk to skeletal creatures called Stalchildren (In life "Captain" Keeta's soldiers) in Ikana graveyard who will share information with the player. The player can also use the disguise of the Captain's hat to issue orders to the Stalchildren to open new areas (Namely a cavern below the graveyard where Link learns the song of storms)
