Below is a listing of all 44 Pieces of Heart available in The Wind Waker. Along with the three hearts Link starts with, and the six obtained from various Bosses, Link can obtain a maximum of 20 hearts.

Locations

Location: Forsaken Fortress
Conditions: None
Heart Piece #1: On the bottom floor of the room with the map in the Forsaken Fortress. In the corner behind the barrels there is a switch; step on it to open the gate of the jail cell and find a chest inside containing the Piece of Heart.
Location: Windfall Island
Conditions: None
Heart Piece #2: On Windfall Island, play Hide-and-Seek with the Killer Bees and win. Ivan will give this Piece of Heart as a reward.
Location: Windfall Island
Conditions: Enough Rupees to play Sinking Ships (10)
Heart Piece #3: Play Salvatore's mini-game Sinking Ships on Windfall Island and beat the game in less than 24 shots. Link will be rewarded with a Piece of Heart.
Location: Pawprint Isle
Conditions: None
Heart Piece #4: Sail to Pawprint Isle and crawl into the dome at the center of the island. Drop into the grotto, and then defeat the Red ChuChus along the path nearby. Open the treasure chest at the end for a Piece of Heart.
Location: Postbox
Conditions: Sort 25 letters for Baito
Heart Piece #5: Play Mail Sorting on Dragon Roost Island and complete the 25 letter sort for Baito. Afterwards, mail the letter to his mother; the closest postbox is outside near the King of Red Lions. She'll soon mail a reply back, and it will contain a Piece of Heart

Conditions: Acquire Farore's Pearl
Heart Piece #6: After finishing the Forbidden Woods and obtaining Farore's Pearl, check a postbox for a letter from the Rito Chieftain. After reading it, Link will receive the Piece of Heart.

Conditions: Acquire Treasure Chart #33
Heart Piece #7: Sail to Five-Star Isles and use Treasure Chart #33 to locate the sunken treasure. Salvage it to receive the Piece of Heart.

Conditions: None
Heart Piece #8: Enter the submarine to the south of the Five-Star Isles and defeat the enemies inside. Open the chest for Piece of Heart #8.
Location: Angular Isles
Conditions: Acquire Treasure Chart #15
Heart Piece #9: Sail to the Angular Isles and use the treasure chart to find the chest. It will contain a Piece of Heart.
Location: Angular Isles
Conditions: None
Heart Piece #10: Climb onto the larger of the islands composing the Angular Isles. Push and pull the blocks around the island to get up to the top. Open the chest at the located here for Piece of Heart #10.
Location: Headstone Island
Conditions: Acquire a Hyoi Pear
Heart Piece #11: Buy some Hyoi Pears and travel to Headstone Island. Use a pear to take control of a Seagull and fly to the top of the island. The Piece of Heart is located here; make sure the seagull grabs it, and then return to controlling Link to claim it.
Location: Diamond Steppe Island
Conditions: Acquire Treasure Chart #23
Heart Piece #12: Make your way over to Diamond Steppe Island and find the patch of light using the treasure chart. Salvage the chest, open it to find this Piece of Heart.
Location: Needle Rock Isle
Conditions: Acquire a Hyoi Pear
Heart Piece #13: Use a Hyoi Pear to take control of a Seagull. Fly it to the top of the island, hit the switch, then return to Link. The flames surrounding a chest will disappear, allowing you to grab the Piece of Heart inside.
Location: Greatfish Isle
Conditions: Acquire the Deku Leaf
Heart Piece #14: Climb up to the top of the small island on Greatfish Isle. Adjust the wind direction and float with the Deku Leaf northwest to a small ledge nearby. There is a treasure chest located here containing this Piece of Heart.
Location: Six-Eye Reef
Conditions: None
Heart Piece #15: Board the submarine to the southwest of Six-Eye Reef and defeat the three Moblins inside. Climb the ladder and open the chest for Piece of Heart #15.
Location: Outset Island
Conditions: None
Heart Piece #16: Visit Orca and accept his challenge of attempting to hit him 500 times with a sword. If Link succeeds, Orca will give him Piece of Heart #16.
Location: Stone Watcher Island
Conditions: Acquire the Cannon
Heart Piece #17: Look for the enemy outlook platform near Stone Watcher Island and destroy the cannons underneath. Climb up the outlook and open the chest that appears for this Piece of Heart
Location: Tingle Island
Conditions: Acquire the Cannon (or Boomerang)
Heart Piece #18: A Big Octo can be found to the north of Tingle Island; defeat it using your cannon or boomerang and then use the Grappling Hook to haul up the treasure where it was killed. The chest contains Piece of Heart #18.
Location: Spectacle Island
Conditions: Enough Rupees to play Barrel Shoot (50)
Heart Piece #19: Visit Spectacle Island and play the cannon shooting game that Salvatore has set up. If Link completes the mini-game, his reward will be a Piece of Heart.
Location: Windfall Island
Conditions: Enough Rupees to win at the Auction House
Heart Piece #20: Take part at the nightly auction on Windfall Island for a chance to bid on this Piece of Heart. Bring lots of Rupees to maximize your chances of winning.
Location: Windfall Island
Conditions: Acquire the Deluxe Picto Box
Heart Piece #21: Take a color photo of Linda and give it to Anton. Go to the cafe later and speak with Linda to discover they are now seeing each other. She'll give Link a Piece of Heart as a reward.
Location: Crescent Moon Island
Conditions: Acquire Treasure Chart #11
Heart Piece #22: On Crescent Moon Island, use Treasure Chart #11 to find the location of this Piece of Heart, then salvage it up from the bottom of the ocean.
Location: Seven-Star Isles
Conditions: Acquire the Cannon (or Boomerang)
Heart Piece #23: Defeat the 12-eyed Big Octo on Seven-Star Isles and collect the treasure that it drops: Piece of Heart #23.
Location: Postbox
Conditions: Acquire 20 Golden Feathers
Heart Piece #24: After giving the guard outside the Rito Chieftain's room twenty Golden Feathers, check the postbox for a letter from his girlfriend. Piece of Heart #24 will be included inside.
Location: Flight Control Platform
Conditions: Acquire the Deku Leaf
Heart Piece #25: Head over to the Flight Control Platform and play the Bird-Man Contest located here. Adjust the wind and use the Deku Leaf to cross the goal at the end. Make sure to utilize the cyclones and have the magic meter upgraded. The brothers will be so impressed that they will reward Link with a Piece of Heart.
Location: Bomb Island
Conditions: Acquire Bombs
Heart Piece #26: Use a bomb to explode the rock in the center of the island and clear the obstacles inside the grotto that appears. Open the chest at the end to get Piece of Heart #26.
Location: Forest Haven
Conditions: Acquire Treasure Chart #31
Heart Piece #27: At Forest Haven, use Treasure Chart #31 to find and pull up a chest containing this Piece of Heart.
Location: Three-Eye Reef
Conditions: Acquire Treasure Chart #38
Heart Piece #28: Sail over to the Three-Eye Reef, find the glowing spot using Treasure Chart #38 and haul it open to obtain a Piece of Heart.
Location: Star Island
Conditions: Acquire Bombs
Heart Piece #29: On Star Island, enter the grotto beneath one of the rocks and defeat the enemies found there. Then, open the chest for Piece of Heart #29.
Location: Windfall Island
Conditions: None
Heart Piece #30: Plant 14 of the plants from Zunari's Shop around Windfall Island. When all the pots are filled, speak with the man on the bench for a Piece of Heart.
Location: Rock Spire Isle
Conditions: None
Heart Piece #31: You can purchase this Piece of Heart from Beedle's Shop Ship near Rock Spire Isle for 950 Rupees.
Location: Rock Spire Isle
Conditions: Acquire the Cannon
Heart Piece #32: Destroy the two ships near Rock Spire Isle and recover their treasures. One of them will be a Piece of Heart.
Location: Southern Fairy Island
Conditions: Acquire Treasure Chart #4
Heart Piece #33: Use Treasure Chart #4 when Link is near Southern Fairy Island. Pull up the chest found here and Link will be rewarded with a Piece of Heart.
Location: Pawprint Isle
Conditions: Acquire Treasure Chart #30
Heart Piece #34: Sail to Pawprint Isle and use Treasure Chart #30 to locate the treasure. Once Link finds it, he will be rewarded with the Piece of Heart.
Location: Greatfish Isle
Conditions: Acquire the Shop Guru Statue
Heart Piece #35: Reward for completing the Trading Sequence with the Traveling Merchants. When Link trades the Shop Guru Statue, his prize will be Piece of Heart #35.
Location: Various
Conditions: Water all of the Withered Trees
Heart Piece #36: Water the final Withered Tree with Forest Water. When Link waters the last one, Piece of Heart #36 will pop out of it.
Location: Windfall Island
Conditions: Acquire Fire Arrows
Heart Piece #37: If you have yet to do so, walk behind Salvatore's mini-game and climb the ladder. Step on the switch here to cause the windmill to start moving. Enter Salvatore's mini-game and climb the stairs on the left to get back outside. Walk around and jump onto the moving windmill. Once it has reached its peak, shoot a Fire Arrow into the spinning center of the windmill. Head back to the way you came out and speak to Kreeb who will give you a Piece of Heart.
Location: Windfall Island
Conditions: Acquire Fire Arrows, Deku Leaf
Heart Piece #38: After lighting the windmill on Windfall Island, use the Deku Leaf to float over to the small island where a chest appeared, which contains a Piece of Heart. This chest can only be opened at night.
Location: Windfall Island
Conditions: None
Heart Piece #39: Speak to Maggie on the top floor of the Auction House and agree to post her Letter to Moe. Do so, and return to the building to start a cutscene. Afterwards, head to the cafe and speak to Ilari to retrieve the Moblin's Letter. Give it to Maggie to obtain a Piece of Heart.
Location: Rock Spire Isle
Conditions: Acquire Treasure Chart #2
Heart Piece #40: Found near Rock Spire Isle, use Treasure Chart #2 to pull up the chest containing a Piece of Heart.
Location: Outset Island
Conditions: Acquire some All-Purpose Bait
Heart Piece #41: Carry the giant black pig all the way across the bridge to the other half of Outset Island. Throw some All-Purpose Bait on the patch of dirt to the left and the pig will dig up Piece of Heart #41.
Location: Bomb Island
Conditions: Acquire Treasure Chart #20
Heart Piece #42: Sail to Bomb Island and use your treasure chart to find the sunken treasure. Haul it up to obtain the Piece of Heart.
Location: Thorned Fairy Island
Conditions: Acquire Treasure Chart #5
Heart Piece #43: Head over to Thorned Fairy Island and find the treasure from Tresure Chart #5, a Piece of Heart.
Location: Two-Eye Reef
Conditions: Acquire Treasure Chart #46
Heart Piece #44: This Piece of Heart can be found within a sunken treasure obtained from Treasure Chart #46. Find it near the Two-Eye Reef and salvage it to get the final piece.
NOTE: If playing on the GameCube version, you can find this Heart Piece after completing all fifty levels of the Savage Labyrinth on Outset Island.
