The Kokiri Sword is an item in Ocarina of Time and Majora's Mask. In both games, it serves as the weakest sword.
Ocarina of Time
" You got the Kokiri Sword! On the Equipment Subscreen, select it with the cursor and equip it with 
. This is a hidden treasure of the Kokiri, but you can borrow it for a while. Be sure to practice with it before you really fight!"
In Ocarina of Time, the Kokiri Sword is found in the Kokiri Forest, specifically the Forest Training Center where Link has to crawl through the small hole to where the Kokiri Sword is hidden. Before Link is able to meet the Great Deku Tree, Link is forced to find the Kokiri Sword and the Deku Shield by Mido. It is then that Link goes to the Forest Training Center and obtains the sword. This sword remains the sword of choice for Young Link throughout the game.

Majora's Mask
"Kokiri Sword The trusty sword you're familiar with that's from Kokiri Forest."
In Majora's Mask, Link already has the Kokiri Sword equipped on him from his quest in Ocarina of Time. Link can later temporarily upgrade it to the Razor Sword in the Mountain Smithy in Mountain Village for 100 rupees. Having done so, he can permanently upgrade it to a greater sword, the Gilded Sword, within the same cycle provided he gets the Gold Dust from the Goron Racetrack before the end of the Second Day.
After Link obtains the Hero's Bow, the Kokiri Sword can be stolen by Takkuri. If so, it can be regained by either using the Song of Time to reset to the First Day, or buying it back from the Curiosity Shop - labelled as "Good Sword" - for 50 rupees.

Cadence of Hyrule

In Cadence of Hyrule, the Kokiri Sword is a Legendary Weapon found in Future World that only Link or Shadow Link can wield. It deals four points of damage per hit, but can only attack the enemy directly in front of him. Upon getting it the Dagger equivalent, the Jeweled Dagger, is unlocked for the other characters.
