The Zora's Sapphire is the last of the three Spiritual Stones needed to open the Door of Time in Ocarina of Time.
Ruto gives it to Link after he has saved her from Lord Jabu-Jabu's Belly. To the Zoras, this Sapphire is the equivalent of an engagement ring. Therefore, the only reason Ruto gives Link the item is because she likes him.
