Mine would be Metroid Prime 2. It was creepy when i saw it the first time, maybe thats why it made such an impact on me to remember.

I played Pac Man ever since I was really little so I died a lot, I even remember first dying on that game. So that is probably why it comes to mind when someone says GAME OVER. And there are lots of versions, where the ghosts and Pac Man come back.

Personally, I love the Castlevania ones. They are often overly creepy and completely irrelevant to anything in the game, but they look awesome.

Case in point: The Portrait of Ruin game over screen. Imagine the words "Game Over" written here in gothic lettering (I couldn't find the image with the text):

Cmon guys, how could none of you have chosen the one I remember the most? Banjo-Kazooie, the game over video was funny as heck 'specially the end with Mumbo.
