I watched a video by NintendoBlackCrisis (https://www.youtube.com/watch?v=oJwllSGUWvY) about malice and came up with this theory. Not sure if its good or not but whatever. You may need to watch the video for context.
Ganon never reincarnated and stayed dead. Malice found either his remains or spirit and used his hatred to fuel him and bring him to "life". The malice grew stronger over those 10,000 years until it thought it was ready to release itself. While releasing itself, it spread (like a disease) to all able hosts, such as Guardians and spirits until it had taken over all of Hyrule.
Just a thought -Why would malice create the eyes if it is indestructible without them? Most malice 'pools' don't have these weak spots so maybe the are hidden from sight beneath the malice? Maybe the eyes are it's true physical form? Who knows...
Also - We're the monsters already in Hyrule or did the malice create them, and if so, how can they drop bones and organs and such when they were created by pure disease? Guess we'll never know...
