Malladus is not a reincarnation of Demise. Demise said that his spirit would follow Link and Zelda forever. Malladus existed before Link and Zelda arrived in New Hyrule.

The original LoZ is pretty much open to you.You can go almost anywhere at anytime. If you don't want to use the walkthrough, then just explore. You're bound to find something eventually.

Also if you go back in time, the great fairies will be split up into the little fairies again, so you have to recollect them all if you didn't get the power up.

I don't think I've ever gotten 100% completion. It's tough to say how far though. I think all of the heart pieces and skultulas, but there are probably other things I have missed.
